- Hours - 32 hours per week with benefits
- Pay Range - $29-$32 per hour
- Location - 1910 County Rd C E, Maplewood, MN 55109

- Assures residents maintain admission criteria, monitor, and recommend alternative placement if necessary.
- Assures residents have up-to-date care plans per state regulations.
- Monitors and maintains resident charts and clinical progress notes.
- Communicates to physicians, staff, and family changes in resident's needs or conditions.
- Facilitates coordination of resident services with other service providers.
- Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
- Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
- Communicates in resident's log any information that staff need to be aware of.
- Provides alternating supervisory visits in coordination with the RN.
- Works as a team player with all members of the residents' care team.
- Supervises medication practices and medical equipment being used in the building.
- Assures that staff clearly understand and follow all resident care procedures.
- Shares resident and family concerns with pertinent staff if appropriate.
- Works with manager to assure emergency procedures are in place.
- Maintains a current state license as an LPN per state regulations.
- Minimum of one year of experience in home health, assisted living, or long‑term care preferred.
- Good organizational skills with the ability to self‑start and follow through with initiatives with minimal supervision.
- Exhibits a caring heart with a passion to care for the elderly.
