• Hours - 32 hours per week with benefits

• Assures residents maintain admission criteria, monitor, and recommend alternative placement if necessary.
• Assures residents have up-to-date care plans per state regulations.
• Monitors and maintains resident charts and clinical progress notes.
• Communicates to physicians, staff, and family changes in resident's needs or conditions.
• Facilitates coordination of resident services with other service providers.
• Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
• Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
• Communicates in resident's log any information that staff need to be aware of.
• Provides alternating supervisory visits in coordination with the RN.
• Works as a team player with all members of the residents' care team.
• Supervises medication practices and medical equipment being used in the building.
• Assures that staff clearly understand and follow all resident care procedures.
• Shares resident and family concerns with pertinent staff if appropriate.
• Works with manager to assure emergency procedures are in place.

• Maintains a current state license as an LPN per state regulations.
• Minimum of one year of experience in home health, assisted living, or long‑term care preferred.
• Good organizational skills with the ability to self‑start and follow through with initiatives with minimal supervision.
• Exhibits a caring heart with a passion to care for the elderly.
