Position Summary

Reporting to the Team Leader or designate, the Community Outreach Nurse assesses, identifies, plans, implements, and evaluates the nursing care required to assist ICM team clients in meeting their physical, social, spiritual and psychological needs. The Nurse works within the team’s interdisciplinary clinical framework to provide client‑centered and strengths‑based care in accordance with the CRNBC/CRPNBC Standards of Practice. The Nurse provides treatment services that are inclusive and flexible for individuals with mental illness, addictions, and/or chronic medical conditions, ensuring integration and continuity across the care continuum.

The Nurse establishes and maintains relationships with, and consults with, service and housing providers, clients, their families, and colleagues.

Hours

67.5 bi‑weekly

Schedule

This position requires the person to work a combination of day (8:30 am – 4:30 pm) and evening (12:30 pm – 8:30 pm) shifts and includes one weekend a month with shift time 8:30 am – 4:30 pm. Schedules are subject to change to meet operational requirements and client needs. Includes a 30‑minute unpaid meal break.

Requirements

Must have a clean driver abstract as well as a valid Class 5 BC driver’s license. The position includes community outreach in Fraser Health Region, light home care if needed, and an individual participant caseload of 8 – 10 that a primary worker oversees.

A clear British Columbia Vulnerable Sector Criminal Record Check for working with vulnerable adults and/or children is a condition of employment. Applicants must be willing and able to receive a clearance under the Criminal Record Review Act.

Transportation

Transit is reality for those living in Vancouver; please note that this is a 2‑hour commute on transit. For those living in the Tri‑Cities/Pitt Meadows/Maple Ridge area, the commute is under an hour depending on where you live. Please be sure you have a reliable source of transportation to make it to work on time.
