The Licensed Practical Nurse (LPN), reporting to the Director of Care, is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with provincial standards of care.

• Work as a member of the inter-professional team, applying a person-centered approach to care.
• Complete and ensure resident's clinical care assessments, care/service plans, and documentation are accurate, up-to-date, and communicated as required.
• Implement and evaluate the effectiveness of care plans, making changes as needed.
• Assist with scheduling resident care conferences and participate when applicable.
• Work closely with the Director of Care to facilitate resident move-ins, move-outs, and transfers.
• Provide direct resident care, medication/treatment administration, and document care in compliance with regulatory standards and policies.

• Current registration as an LPN in the applicable Province/State.
• Experience in a senior's environment and with unregulated care providers preferred.
• Education and experience in gerontology, rehabilitation, community/long-term care are assets.
• Strong communication, critical thinking, care coordination, time management, and customer service skills.
• Demonstrated sound clinical knowledge.
• Current Basic Cardiac Life Support and First Aid certifications.
• Proficiency with Microsoft Office applications.
• Ability to provide or obtain a current (within six months) Vulnerable Sector Check, including a Criminal Background Check.

Salary: $32.84 - $41.35 per hour, based on qualifications and experience.
