Duties and Services- Individualized pharmacotherapy
- Counselling patients about their medication therapy
- Work in collaboration with physicians and other health‑care providers to develop, implement and monitor therapeutic plans
- Assist in the development of drug‑therapy guidelines for individual disease conditions
- Prepare and submit case studies, articles and research for publication in professional journals
- Act as a department resource person within your assigned area
- Participate in medication‑use evaluations, quality‑improvement initiatives and clinical drug trials
- Provide mentorship and supervision to pharmacy students, residents and new staff
- Ensure prescriptions are validated, dispensed accurately and that medication safety concerns are resolved expediently; document all professional practice activities proficiently
- Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
- Completion of an accredited Hospital Pharmacy Residency Program.

Detailed OverviewSituated within identified Correctional Health Services locales, this position provides clinical pharmacy services to clients of the Correctional Health Services. As a member of an interprofessional team, you will work directly with clients to identify and resolve actual and potential drug‑therapy problems through comprehensive medication management.
