Peer Recovery Support Specialist​/Case Manager - Manteno

Job Description

$2,000 SIGN ON BONUS

1st $1,000.00: after 30 days of employment

2nd $1,000.00: after 6 months of employment

• Facilitate linkages/coordinate services with outside agencies for patients with unmet needs (i.e., childcare, transportation service, psychiatry, housing, etc.)
• Track attendance of newly enrolled patients to ensure they complete orientation within allotted days of admission
• Review FGC program policies in group or individual sessions
• Compile data to present to medical team, in conjunction with clinical staff input, when doctor intervention is indicated
• Attend to patients in crisis at the request of medical reception or other staff

Work Schedule: Monday - Friday (8:00am - 4:30pm)

Start Date: ASAP

Work Location: 333 W. Evergreen St., Manteno, IL

Pay Rate: Up to $18.00 per hour; based on experience

DCFS clearance required.

Education and/or

Experience:

High School Diploma/GED required;
Experience in/with substance use, detox programs, medication-assisted treatment, behavioral healthcare and/or community relations encouraged to apply.

Certificates, Licenses, Registrations: CADC or CPRS preferred
