• Position Type:
  Non-exempt, full-time or part-time, hourly role.
• Scope:
  Responsible for providing direct support within a single program or unit, typically supporting 4 - 6 individuals in residential settings, or up to 10 individuals in periodic programs.
• Direct Support:
  Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Daily
  
  Activities:
  
  Assist with personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Health Care and Records:
  Accurately document progress and activity; review records and logs to stay informed of changes in service plans; maintain confidentiality.
• Asset Management:
  Make cost-effective and appropriate purchases within the budget of the individual served and according to their choice/preference.
• Relationships and Reporting:
  Maintain professional relationships with individuals, families, guardians and case managers; greet visitors; report any abuse/neglect and medical or behavioral incidents per Sevita policy and external requirements.
• Rights:
  Maintain confidentiality and respect the rights of persons served; practice universal precautions; assist individuals in exercising their rights.

• Appointments:
  May accompany individuals to medical appointments; relay orders and information to/from medical providers as required.
• Medications:
  If assigned, administer or support self-administration of medications; document delivery; report administration errors; maintain security of medications and supplies.
• General Health Care:
  Monitor health, document concerns, communicate with nurse or supervisor; call 911 in an emergency; follow health care directives.
• Medical Equipment and Meals:
  Follow procedures for using medical equipment; report malfunctions; assist with meal preparation and adherence to planned menus and special diets; monitor mealtime and dining plans.

• Training:
  Attend orientation and ongoing training; participate in monthly staff meetings.
• Scheduling:
  Work scheduled hours; arrive on time; stay on shift until replacement arrives; notify supervisor of absence; may fill in for other shifts as needed.
• Teamwork and Safety:
  Work with others, communicate openly, show respect; maintain a safe workplace and report hazards; participate in safety training.

• Vehicles and Transportation:
  May transport individuals; drive safely; use safety equipment; report accidents and safety concerns.
• Housekeeping and Maintenance:
  Maintain a clean, safe environment; report hazards; perform minor repairs as needed; assist with yard work and snow removal.
• Safety:
  Check water temperature when assisting with bathing; participate in safety drills and protect individuals in emergencies.

• Performs other duties as required.
• MPA = Mobile Punch Authorized for timekeeping.

• Education and Experience:
  
  High school diploma or equivalent preferred; six months of experience in human services preferred; must be 18 years of age; working knowledge of computers.
• Certificates, Licenses, and Registrations:
  Valid driver’s license in good standing; car registration and vehicle insurance if providing transportation; all state-required trainings and certifications completed within mandated time frames; maintain license and be approved to drive; comply with driving-record requirements.

• Work Location:
  
  Program location with possible community travel.
• Physical:
  Heavy Work; exerting up to 100 pounds occasionally, up to 50 pounds frequently, and up to 20 pounds constantly.

• Additional requirements:
  
  cooperation with licensing and DHS staff, valid NJ driver’s license; basic educational requirements; drug and background checks as required; required trainings; minimum age 18.
