As a Behavioral Health Case Manager at Noble, you will:
- Deliver Compassionate Care:
Provide patient-centered case management and resource navigation to support and improve overall well-being. - Be Part of an Integrated Care Model:
Deliver evidence-based, quality care with the support of a collaborative team, including Medical, Pharmacy, and Dental providers. - Serve as a Case Management Expert:
Work closely with an interdisciplinary team while offering your expertise to address diverse needs. - Expand Access to Care:
Help break barriers to care by focusing on the needs of underserved communities. - Innovate and Collaborate:

Mentor and collaborate with students training at Noble, contributing to the growth of the next generation of care providers.
00ll Do
- Provide case management and follow-up care to support patients in meeting health and treatment goals.
- Collaborate with primary care and behavioral health providers to coordinate care, treatment plans, and patient education related to mental health, SUD, and psychosocial needs.
- Serve as a liaison across the integrated care team to improve communication and coordination of services.
- Assess Social Determinants of Health (SDoH) and connect patients to appropriate internal and community resources.
- Advocate for patients to help navigate systems and secure needed services.
- Support patient re-engagement and care coordination activities.
- Conduct secondary suicide risk assessments, develop safety plans, and assist with crisis de-escalation using trauma-informed approaches.
- Facilitate integrated care clinical consult team meetings.
- Master’s degree in Social Work, Counseling, Marriage and Family Therapy, Psychology, or a related behavioral health field from an accredited program
- Current Wisconsin licensure in one of the following, or eligibility under a valid WI training license:
- Licensed Clinical Social Worker (LCSW) or APSW
- Licensed Professional Counselor (LPC) or LPC-IT
- Licensed Marriage and Family Therapist (LMFT) or MFT-IT
- Licensed Psychologist
- For training license holders: active progress toward full licensure, including completion of required 3,000 supervised clinical hours
- Active license in good standing with the State of Wisconsin
The National Health Service Corps (NHSC) Loan Repayment Program (LRP) offers primary medical, dental, and mental and behavioral health care clinicians the opportunity to have their student loans repaid while earning a competitive salary in exchange for serving in urban, rural, or tribal communities with limited access to care. State of Wisconsin loan repayment opportunities may also be available.
