• Provide high-quality social work support to adults in the community, with a focus on older persons.
• Carry out comprehensive assessments and develop person-centred care plans.
• Manage and review caseloads to ensure safe and effective interventions.
• Work collaboratively with families, carers and partner agencies.
• Maintain accurate, timely and compliant case records.
• Use Liquid Logic to record assessments, plans and outcomes effectively.

• Qualified Social Worker with registration to the relevant professional body.
• Minimum two years’ post-qualification experience in adult social care.
• Experience in community-based adult services.
• Working knowledge of Liquid Logic preferred.
• Strong assessment and care planning skills.
