Salary: £35,435.97 per annum

WCHG is recruiting a number of Living Well Coordinators, both on fixed‑term contracts or as permanent positions. This is a rewarding opportunity to join our values‑driven team and deliver tailored support services that empower residents, improve wellbeing, and promote tenancy sustainment. Please note that fixed‑term positions will be contracted until 31st March 2027.
AboutThe Role
Working within a defined locality, you’ll be the key contact for tenants and residents who require more intensive, person‑centred support. From safeguarding and health challenges to financial difficulties or tenancy risks, you’ll coordinate multi‑agency responses and work collaboratively with both internal teams and external partners to help people overcome barriers and build brighter futures.
What You'll Be Doing- Deliver high quality, trauma‑informed support to individuals and families
- Address issues such as tenancy breakdown risk, domestic abuse, mental health, hoarding, social isolation and poverty
- Coordinate support and interventions across agencies including social care, health, housing and the voluntary sector
- Develop and manage individual support plans, prioritising resilience and independence
- Support and coach individuals through complex circumstances, including safeguarding concerns
- Maintain accurate records, report outcomes and contribute to service improvement
- Act as a Designated Safeguarding Officer as part of a rota
- Experience working with vulnerable people with complex needs
- Confident in safeguarding procedures, tenancy‑related support, and collaborative casework
- Strong understanding of housing, health, domestic abuse, and welfare issues
- Excellent interpersonal and organisational skills
- Solution‑focused, emotionally resilient and empathetic
- Ability to manage a caseload and work both independently and in teams
- Comfortable using IT systems and maintaining detailed records
- A full UK driving licence is desirable
