- Qualification - Social Work Qualification and registration with Social Work England
- Essential Car Documentation - This post is a designated Essential Car User. Therefore, you must always hold a full valid driving licence and provide your own car for use will receive the Essential Car User Allowance.
- Medical Clearance - Pre-placement health assessment (PPHA)
- DBS (Disclosure and Barring Service) clearance - this post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended in 2013). Appointable candidates will be required to apply for an Enhanced Disclosure and Barring Service (DBS) check. Information provided by you or the DBS will be dealt with in a confidential manner and in accordance with the DBS Code of Practice which can be viewed here.

We are looking for a Senior Social Worker for the North Manchester Community Hospital Discharge To Assess Community team. The focus of the role is to achieve outstanding outcomes for all of our citizens requiring social work assessment and care planning following discharge from Acute bed.
This is a busy and focused team and the role holder should be a critical thinker, with the ability to manage risk and need, whilst maintaining flow through the system. HDTA north are a stable and fully staffed team and we are looking for a committed SSW to join our management team.
About the CandidateThe successful Candidate should be skilled, experienced and have good working knowledge of Social Work procedures within Hospital Discharge to Assess.
They should have both formal and informal experience of supervising social workers and care assessors. Candidates should have the BIA or Practice Educator qualification, or be willing to work towards this.
They should be a competent user of Liquid Logic, have generally excellent IT skills across a range of systems.
