The Northern Care Alliance are delighted to offer the opportunity to become our Lead Practice Nurse with The Northern Care Alliance NHS Foundation Trust.

The post holder will be responsible for delivery of safe and effective nursing care to the population of patients who live in Nursing and Residential homes in Salford.

This is a service across the city of Salford.

As the Lead Practice Nurse the post holder will account for the nurses delivery service, including leading and managing all nursing resources working closely with GP's and the practice manager to deliver practice priorities.

The role includes leading on the provision of specialist clinical nursing care, health promotion and chronic disease management to the patients registered with the practice. The post holder will lead on the delivery of the indicators in the Salford Standard, Quality and Outcomes Framework and NHSE KPI's ensuring the practice meet requirements.

Clinical Leadership & Patient Care

• Chronic Disease Management: Leading nurse-led service for long-term conditions, managing care plans, and performing reviews.
• Preventative Care: Delivering and leading immunisations, cervical cytology, and chronic disease.
• Clinical Expertise: Providing general nursing care, fall assessment, wound management, and ear syringing.
• Evidence-Based Practice: Implementing and evaluating care based on NICE guidelines and best evidence.

• Experience as a practice nurse
• Non-medical prescriber
• Registered Nurse with current NMC registration (Part 1 Adult / General Level 1 or
  2)
• Degree / Diploma in Nursing studies or equivalent
• Community experience with older adults
• Experience in a leadership role
• Leading MDT
