Job Summary
NTGP has a fantastic opportunity for a Practice Nurse to join our multi‑disciplinary team. The position will require sessions from both locations (on different days) and we are flexible with the days available to suit you, the team and patients. The Practice Nurse will act autonomously within their professional scope of practice, providing care for patients face‑to‑face, telephone consultations and home visits.
You will be responsible for the provision of medical services to our patient population, managing a caseload that deals with a wide range of health needs, supported by the wide range of clinical and non‑clinical team within the practice.
We are a large, forward‑thinking practice of 15,000 patients, providing quality health care and outcomes for our patients in the North Trafford area. We operate from two sites:
Chester Road and Seymour Grove. The team at NTGP comprises GP Partners, 8 salaried GPs, a Physician Assistant, Pharmacists, nursing team and a non‑clinical team managed by the Business Manager. Our practice has been rated “Good” by CQC and we take pride in delivering quality care and maintaining a strong training and personal development ethos. We are part of the North Trafford Primary Care Network.

- Practice independently in accordance with NMC code of conduct and with reference to practice guidelines.
- Maintain accurate documentation of care and utilisation of IT.
- Ensure continuation of own personal and professional development, facilitated by utilisation of clinical supervision networks.
- Provide focus for nursing expertise within practice, advising nursing staff according to level of competence.
- Administer adult and child immunisations and vaccinations in accordance with national and local programmes; be competent in anaphylaxis and resuscitation techniques; ensure safe storage, rotation and disposal of vaccines, and maintain adequate stock levels.
- Takes a lead in areas such as Asthma and COPD management, undertakes the establishment and running of disease management clinics, and provides specialist nursing care for the patients or communities that the practice serves.
- Manage chronic diseases including asthma, hypertension, COPD, CVA/TIA, CHD and CHD risk assessment and management; initiate diagnostic processes for patients suspected of chronic disease.
- Maintain disease registers.
- Initiate and carry out programmes of health promotion and screening, including cervical and sexual health screening.
- Act as a coordinator for the Nurse Lead for Infection Control.
- Qualified Nurse.
- Registered with the NMC.
- Experience working within a nursing team.
- Experience dealing with vulnerable patients.
- Experience working in primary care (Desirable).
- Experience working in a GP practice (Desirable).
