An exciting opportunity has arisen for an experienced and motivated Practice Nurse to join the friendly and forward-thinking nursing team at Bunbury Medical Practice.
We are looking for a highly skilled, autonomous Practice Nurse who is confident in managing long-term conditions, undertakingcervical cytology and delivering childhood and adult immunisations. You willplay a key role in delivering high-quality, patient-centred care to our registered population of approximately 5,500 patients.
You will work closely alongside Advanced Nurse Practitioners, GPs, HCAs and the wider multidisciplinary team, in asupportive environment that values clinical expertise, continuity of care and professional development.
Main duties of the jobThe following arethe core responsibilities of the Practice Nurse.
Assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
Implement and embed health promotion and wellbeing programmes
Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health
Provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
Diagnose and manage acute and chronic conditions
Prescribe medication for patients in accordance with PGDs and own scope of practice
Review patient medications to enhance compliance
Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
Maintain accurate clinical records in conjunction with extant legislation
Toensure compliance with the practice chaperone policy
Toassist GPs with minor surgery when required
Tosupport the team in dealing with clinical emergencies
To implement vaccination programmes for adults and children
Tosupport the clinical team with all safeguarding matters, in accordance withlocal and national policies
Todeliver opportunistic health promotion where appropriate
About usBunbury Medical Practice is a welcoming, high-achieving rural practice located in a picturesque village near Tarporley, with the added benefit of an onsite dispensary.
We are led by 2 GP Partners and 4 Salaried GPs and are proud of our strong, multidisciplinary clinical team, which includes:
- Advanced Nurse Practitioners
- Practice Nurses and HCAs
- First Contact Physiotherapist
- Dementia Nurse
We are also an active member of the Rural Alliance Primary Care Network, working collaboratively with five neighbouring practices to enhance patient care and staff development.
Job responsibilitiesTo be responsible for the implementation of processes for the effective management of patients with long-term conditions usingevidence-based practice including care for elderly and housebound patients.
Working as a member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively tothe entitled patient population.
The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, well womenand well man clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.
Bunbury Medical Practice must ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.
Person Specification Qualifications- Registered Nurse (Nursing and Midwifery Council)
- Nurse Prescriber
- Experience working within a primary or secondary care environment
- Experience of chronic disease management
- Experience of working autonomously
- Experience of working as a practice nurse or community nurse
- Experience of infection prevention and control measures
