An exciting opportunity has arisen for an enthusiastic and motivated Band 5 Community Nurse to join the Adult Community Learning Disability Team (CLDT) in Trafford, based at Sale Waterside.
OverviewWe are a well‑established and friendly team who support patients with a learning disability who are registered with a GP in Trafford. You will be part of a supportive and knowledgeable multi‑disciplinary team and have access to an excellent training and preceptorship programme.
Responsibilities- Manage a caseload under supervision, working autonomously and as part of the team to improve outcomes for adults with Learning Disabilities within the local community while maintaining quality of life and ensuring people are free from harm.
- Plan, deliver and evaluate nursing care in line with person‑centred care principles.
- Collaborate with a range of professionals, agencies and carers in a wide variety of settings.
- Demonstrate enthusiasm for community nursing and a commitment to ongoing professional development.
- Band 5 registered nurse with experience in a community learning disability setting (desired).
- Excellent interpersonal, communication and literacy skills.
- Ability to work with a range of professionals, agencies and carers across multiple settings.
- Demonstrated enthusiasm for community nursing and a desire for ongoing professional development.
- Training opportunities, regular clinical supervision and annual appraisal.
- One fixed start date each month aligned with our Prepare to Care (P2C) induction programme, which offers up to three weeks of training.
