Senior Practitioner - Home Engagement and Rehabilitation Team (HEART)
An exciting secondment opportunity has become available, for the role of Senior Practitioner with Home Engagement and Rehabilitation Team (HEART). HEART is an integrated team, comprised of a variety of professionals who offer effective, goal orientatied, person-centred care daily.
This role presents a unique opportunity to provide strong and consistent clinical leadership to a dynamic and enthusiastic team who provide care and support for adults with complex and enduring mental health needs.
The desired individual is a professional who is motivated, driven, and has the enthusiasm and ability to provide clinical oversight for the team, and support individuals within the team via supervision.
We are looking for somebody who is passionate about rehabilitation, and particularly about the development of community rehabilitation services as part of wider transformation efforts across the mental health landscape. Furthermore, we are seeking somebody with the necessary skillset to support integration with inpatient, urgent care, local authorities, acute services and the VCSE sector
The role of HEART senior practitioner will enable you to support the development of staff and the service delivery more broadly. There is huge potential and scope to genuinely make a difference to the lives of the service user group.
Main duties of the jobTo be responsible for the day-to-day clinical leadership of a multi-disciplinary community mental health rehabilitation team. To ensure the team is integrated, flexible, user-focused, and recovery-oriented to deliver a safe, effective, and high-quality service to service users with mental health needs.
Participation as a member of the division senior management team, including deputising for the team manager when required
Participate in the service bronze on site / on call rota as required.

Saturday (midnight to midnight) and any week day after 8pm and before 6am
All time on Sundays and Public Holidays (midnight to midnight)

- Qualified professional (Mental Health Nurse/Social Worker/Occupational Therapist) with current registration with relevant professional body.
E.g. NMC, SWE, HCPS - Significant post qualifying clinical experience of working within the mental health field

- Experience of working within mental health rehabilitation services
- Sound knowledge of the Community Rehabilitation Model of intervention
- Sound knowledge of the current local and national factors influencing mental health rehabilitation
- Sound knowledge of the Care Act, Mental Health Act and Mental Capacity Act
- Knowledge of service monitoring and evaluation
- Knowledge of change management
- Ability to communicate in a variety of settings and form excellent working relationships
- Line management and clinical supervision skills
- Ability to work to prioritise and work to deadlines
- Comprehensive formulation of reports and presentations
- Commitment to effective teamwork and service development
- Flexible approach/creative thinking
