Job Overview

An exciting opportunity has arisen for an Adults Social Worker to join the Integrated Neighbourhood Team in Manchester.

This ongoing locum position offers a competitive hourly rate of £30.00 with the flexibility of part-time or full-time working
. You will be delivering high-quality, person-centred social work within a multidisciplinary neighbourhood team, supporting adults with complex needs to live as independently and safely as possible.

This role is ideal for a social worker who enjoys varied, fast-paced frontline practice, multi-agency working, and making a direct impact in the community

What You Will Do

Manage a diverse caseload of adults with complex needs
, including older adults, people with physical disabilities, learning disabilities, and mental health needs.

Complete Care Act assessments and support plans using a strengths-based, person-centred approach.

Lead and contribute to safeguarding enquiries and multi-agency safeguarding processes
.

Undertake Mental Capacity Act assessments and Best Interests decisions
, including involvement in DoLS or LPS processes where required.

Work closely with health colleagues to support admission avoidance, discharge planning, and community-based interventions.

Promote independence and wellbeing through the use of community resources and preventative services.

Coordinate complex care packages and commission services in line with council procedures.

Participate in multi-disciplinary meetings, case discussions, and care planning forums
.

Maintain high-quality, timely, and accurate case recording in line with statutory and local requirements.

Requirements

Degree or equivalent qualification in Social Work.

Registered with Social Work England
.

Completion of ASYE or equivalent post-qualifying experience
.

Experience working in Adult Social Care
.

Good working knowledge of:

Care Act 2014

Mental Capacity Act 2005

Safeguarding Adults procedures

Ability to manage complex cases, work autonomously, and operate effectively within a multidisciplinary environment
.
