Overview
The School Health Services, comprising School Nursing and Healthy Schools, operate citywide across Manchester to improve the health outcomes of children, young people and their families. The Services include School Health Screening, Healthy Schools, Healthy Weight Team and the Healthy Child Programme (HCP) and address key public health priorities for children.
Responsibilities- Provide a public‑health‑based service that improves the health of children and families through partnership working in a multi‑agency arena.
- Work in partnership with families and other agencies to contribute to the assessment framework for children in need/early help assessment processes.
- Assess, plan, deliver and evaluate the health needs of specific groups of children (e.g., looked‑after children, children with additional needs) within the scope of professional practice using appropriate frameworks.
- Coordinate and manage own workload, including data collection, record maintenance and reporting mechanisms.
- Complete health assessments for children and young people as required.
- Support the SEND agenda and EHC by providing relevant information to compile care plans and by coordinating care‑plan involvement for children in school or other settings.
