Overview
This is an opportunity to be part of a dedicated team providing exceptional care to children and families across the city. In this role, you will lead and attend to the diverse health needs of children in Manchester, including those facing end-of-life challenges and with complex conditions. You will work with the Head of Service to apply your clinical expertise to impact patient outcomes, with day-to-day leadership and management responsibility for a dedicated children’s service and a designated paediatric caseload.
You will operate as an autonomous clinician in a defined area to develop and provide expert clinical advice, support and care to a defined group of patients.
- Oversee and undertake completion of children’s continuing care assessments and collaborate with commissioners to offer appropriate care provision.
- Monitor and review the implementation of treatment plans, making changes within agreed protocols, pathways or models for clinical effectiveness.
- Empower children and young people and their families/carers through appropriate support and education.
- Establish and maintain communication networks within primary, secondary and tertiary care settings to ensure multi-disciplinary and multi-agency training programmes.
- Participate in induction and orientation programmes for newly appointed staff.
- Maintain and develop expert knowledge and clinical skills relating to children’s complex care through relevant training and development.
- Maintain high standards of professional practice and competence in line with the NMC Code.
- Engage in personal development planning with the line manager and facilitate individual and team objectives.
- Participate in leadership activities and contribute to the leadership of self and the team in line with the Trust Supervision Policy to improve quality of care.
- Educate, manage, support and mentor groups and individuals on best practice in relation to children’s complex care.
- Provide advanced clinical knowledge within your scope of practice to other professionals.
- Contribute to professional and policy-making activities that impact the development of clinical practice.
- Lead and manage the children’s community complex care service; delegate patient care to appropriate staff to provide a safe and effective service.
- Participate in and contribute to leadership and management responsibilities in the absence of other team leaders and/or Head of Service.
- Take a lead role in evaluating clinical practice to identify and implement changes to continuously improve standards of clinical care.
- Contribute to the development of the children’s community nursing team.
