Specialist Learning Disability Liaison Nurse

Northern Care Alliance (Primarily Fairfield & Royal Oldham Hospitals)
Make a difference for patients with learning disabilities across our hospital network.

We're looking for a compassionate and skilled Specialist Learning Disability Liaison Nurse to join our dedicated team. This role is part of the wider Learning Disability Liaison Team, supporting patients of all ages across the four Northern Care Alliance hospital sites.

What you'll do
  • Work collaboratively with patients, families, carers, and healthcare professionals
  • Support senior specialist nurses to ensure care and treatment for patients with learning disabilities is well-planned and delivered
  • Coordinate admissions and monitor key patient groups requiring day-case interventions, including case management responsibilities
  • Provide support for both adults and children accessing planned or emergency admissions
  • Primarily work within Fairfield and Royal Oldham Hospitals, with occasional cross-site cover as part of a planned approach
  • Champion patient safety and positive experiences at every stage

Main duties of the job

We're seeking a dedicated Specialist Learning Disability Liaison Nurse to join our trust-wide Learning Disability Liaison Nursing team. This is a unique opportunity to influence positive change and improve patient experiences across the Northern Care Alliance.

What you'll do
  • Provide expert knowledge and guidance to acute service teams, supporting the implementation of best practice for patients with learning disabilities
  • Deliver specialist care across the NCA footprint, focusing primarily on Bury and Oldham areas with planned cross-site cover
  • Work as part of a multidisciplinary team, advising on reasonable adjustments and contributing to audits and service development
  • Support patients of all ages, ensuring safe, inclusive, and person-centred care

Person Specification Qualifications & Knowledge, Training & Experience
  • Registered Nurse in Learning Disabilities (RNLD)
  • Nursing degree or equivalent
  • Knowledge of the Mental Capacity Act
  • Working as part of a team
  • Professional Self Development
  • Knowledge of Acute Services
Skills & Abilities
  • Excellent interpersonal skills
  • Clear written and verbal communication
  • Clear evidence of multidisciplinary working
  • Ability to adapt communication
  • Knowledge and implementation skills of the mental capacity act
  • Experience of case hold management
  • Ability to travel to different geographical locations across the Trust footprint in line with service needs
  • Experience at delivery of training
  • Full UK driving license with willingness to use own car for work purposes
