The Salford Memory Assessment and Treatment Team (MATS) are pleased to offer a full time Band 6 position within the team.
The successful applicant will provide specialist community and clinic-based memory assessments and support the team to deliver post diagnostic support including treatments. The team is based at Woodlands Hospital in Little Hulton.
The successful applicant will also cover duty, completing the screening of referrals, offering advice and signposting into appropriate services and deal with any general enquiries.
There is an expectation for the band 6 nurse to supervise junior staff and support students whilst on placement with the team.
Applicants should have a good foundation of dementia knowledge and it would be beneficial to have some experience in older adult community care.
The post holder will be required to complete home visits ability to travel the Salford footprint is essential.
Main duties of the jobThe main focus of this role will be in memory assessment. The team provides early identification of a dementia or cognitive impairment by means of memory assessment and cognitive screening. Band 6 nurses will learn skills in delivering a diagnosis and oversee the band 5 nurses who support initiation of treatments. There will also be opportunities to be involved in short term post diagnostic support and relevant research.
Knowledge of dementia treatments and current advances in dementia care and treatment is useful, however a passion for the speciality and motivation to learn and develop with the team is expected.
Band 6 staff are allocated a duty day where they screen referrals, offer advice and signpost. Competent communication skills are required to liaise with GPs and other referrers to ensure appropriate referrals are accepted alongside maintaining good working relationships with our wider NHS and social care colleagues.
The successful candidate will also be expected to contribute to the wider working of the MDT by participating in the weekly MDT meetings and will undertake supervision of junior staff and trainees, as delegated by the MATS Team Manager.
In the absence of senior staff there maybe an expectation to complete some leadership tasks depending on experience.
