Team Leader - Children and Young People Complex Care

This is an opportunity to be part of a dedicated team providing exceptional care to children and families across the city. In this role, your autonomy will shine as you lead and attend to the diverse health needs of the children in Manchester, including delivering exceptional care to those facing end-of-life challenges and with complex conditions. Working in conjunction with the Head of Service, you'll leverage your clinical expertise to make vital decisions that directly impact patient outcomes.

You will have day-to-day leadership, management and development responsibility of a dedicated children's service and to hold a designated paediatric caseload. To work as an autonomous clinician in a defined area to develop and provide expert clinical advice, support and care to a defined group of patients. To oversee and also undertake the completion of the children's continuing care assessments, work in collaboration with the commissioners to offer appropriate care provision.

Monitor and review the implementation of treatment plans and make changes within agreed protocols, pathways or models for clinical effectiveness. Empower children and young people and their families/ carers through appropriate support and education. To establish and maintain individual and team communication networks within primary, secondary and tertiary care settings to ensure multi‑disciplinary and multi‑agency training programmes.

Main duties of the job

Participate in the induction and orientation programmes for newly appointed staff.

Maintain and develop an expert knowledge base and clinical skills, through relevant training and development relating to children's complex care.

Maintain high standards of professional practice and competence in line with the NMC Code. Participate in personal development planning with line manager and facilitate individual and team objectives. Participate in and contribute to the leadership of self and of the team according to the Trust Supervision Policy in order to improve quality of care. Educate, manage, support and mentor groups and individuals on best practice in relation to children's complex care.

Provide advanced clinical knowledge within own scope of practice to other professionals. Contribute to professional and policy making activities that impact on the development of clinical practice. To lead and manage the children's community complex care service; delegating patient care to appropriate members of staff, to provide a safe and effective service. Participate in and contribute to the leadership and management responsibilities in the absence of other team leaders and/or Head of Service.

Take lead role in the evaluation of clinical practice to identify and implement change to continuously improve standards of clinical care.

To contribute to the development of the children's community nursing team.
