Overview

The Salford Memory Assessment and Treatment Team (MATS) are pleased to offer a full time Band 6 position within the team. The successful applicant will provide specialist community and clinic-based memory assessments and support the team to deliver post diagnostic support including treatments. The team is based at Woodlands Hospital in Little Hulton.

Responsibilities
  • Provide specialist memory assessments in community and clinic settings.
  • Support post-diagnostic care including treatments; contribute to MDT meetings.
  • Cover duty by screening referrals, advising and signposting to appropriate services, handle general enquiries.
  • Supervise junior staff and support students on placement; participate in leadership tasks as delegated.
  • Contribute to wider MDT work, participate in weekly MDT meetings, and liaise with GPs, referrers and NHS/social care colleagues.
  • Possibly deliver a diagnosis and oversee Band 5 nurses supporting initiation of treatments; opportunities for short-term post-diagnostic support and involvement in relevant research.
  • Fulfil duties during absence of senior staff as appropriate.
Qualifications/Experience
  • Good foundation of dementia knowledge; beneficial to have experience in older adult community care.
  • Ability to travel within Salford footprint for home visits; full driving licence may be required.
  • Experience in mental health, learning disability or drug and alcohol services may be relevant.
  • Strong communication and liaison skills with GPs and referrers; ability to work with diverse teams.
  • Desire to learn and develop with the team; knowledge of dementia treatments and current advances is useful.
