Position: Community Occupational Therapist - Hospital Home | University NHS Foundation Trust

Overview

Hospital @ Home a 7 day service. This role entails working some weekends, bank holidays and shift work on a rota basis. The post involves working within the community, delivering holistic assessments & interventions to patients in their own homes/place of residence to prevent unnecessary admission to hospital. Independent travel within the local geographical area to deliver the role is essential.

With the above in mind, you must have experience of working in the NHS and in community services. The successful candidate should have at least 3 years post registration experience and have the ability to assume a Team Lead role. The Team Lead role is integral to providing adequate leadership to support the multidisciplinary team and to ensure service development in line with Urgent care requirements.

Previous leadership experience would be beneficial for the ideal post holder.

The clinician will be able to demonstrate an appropriate level of clinical skills in occupational therapy, will be experienced in administering person-centred assessments, as well as implementing and evaluating occupational therapy interventions in a fast paced working environment. You must have a recognised Occupational Therapy qualification and current HCPC registration.

Responsibilities
  • Initiate history taking, clinical assessment, diagnosis and the implementation of a range of care interventions for patients presenting with undifferentiated and undiagnosed problems.
  • Make onward referrals to other services and/or agencies for individuals with ongoing needs.
  • Provide leadership, education and support to other team members through promotion of innovative strategies to manage change and improve service delivery.
  • Provide highly specialist knowledge and clinical skills to ensure the delivery of care within the service and in the wider community and across the acute/community interface.
  • Collaborate with MCR services, NWAS, Primary Health Care Teams, Neighbourhood Teams, Social Care and Voluntary sectors to ensure patients can be managed safely at home to avoid unnecessary hospital admission.
Qualifications
  • Recognised Occupational Therapy qualification and current HCPC registration.
  • Experience of working in the NHS and in community services.
  • At least 3 years post-registration experience; ability to assume a Team Lead role (leadership experience beneficial).
  • Band 7 practitioner level with autonomy within agreed guidelines.
