Job Summary

Deliver care and create a therapeutic environment for patients and families whose needs range from basic to complex by utilizing the nursing process. Carry out functions consistent with Performance Improvement and in accordance with hospital policies and procedures. Demonstrate clinical excellence through proficient practice that enhances and enriches the practice environment.

Responsibilities
  • Assesses patients’ physical and psychosocial status based on interview, examination and review of clinical data.
  • Assesses potential for crisis situations and intervenes before a crisis develops.
  • Reassesses patient status at established time frames and as indicated by the patient’s condition.
  • Based on assessment, analyzes relevant patient data and identifies nursing diagnoses/patient care problems.
  • Communicates patient status and assessed needs to members of the multidisciplinary team.
  • Develops an individualized plan of care, including developmental and cultural needs, in collaboration with the patient, family and interdisciplinary team.
  • Reviews and updates the plan of care as needed.
  • Acts as a resource to others in developing and implementing the plan of care.
  • Uses the nursing process as a basis for developing, implementing and evaluating nursing interventions.
  • Provides patient‑centered care with sensitivity and respect for the wealth of human experience.
  • Performs technical skills safely and competently as required by the clinical unit.
  • Coordinates and implements medical diagnostic and therapeutic orders as directed.
  • Anticipates and manages complications of disease progression and invasive procedures.
  • Documents plan of care, assessments and interventions and patient outcomes in an electronic medical record thoroughly and accurately.
  • Reports status, including response to care and changes in status as appropriate.
  • Ensures a calm, supportive and therapeutic environment for patients and their families, serving as an advocate when appropriate.
  • Adheres to all safety, organizational and ethical standards according to ANA Code of Ethics.
  • Implements elements of core measures in conjunction with Performance Improvement.
