Job DescriptionPOSITION SUMMARY:
To plan, implement, and evaluate nutritional therapy for adults participating in the Cardiac Rehabilitation, Cholesterol Management Center, Outpatient Diabetes Resource Institute, and/or the Wellness Center programs. To plan, implement, and evaluate educational sessions relative to nutrition, weight, cholesterol, and blood glucose management. To collaborate with interdisciplinary staff in monitoring patient progress toward desired outcomes. To conduct community nutrition education courses.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Assesses each referred Cardiac Rehabilitation, Cholesterol Management Center, Outpatient Diabetes Resource Institute, and/or Wellness Center participant for desired weight, blood glucose, and lipid management.
- Demonstrates an understanding of the cardiovascular disease process relative to nutrition, weight, blood glucose, and lipid management and develops a dietary prescription that will safely and effectively guide the patient/client toward an optimal outcome.
- Conducts interview/intake sessions on patients/clients, obtains a diet history, and analyzes the patients’/clients’ diets.
- Reviews patient/client records regarding medical/health history, cardiac and diabetes risk factors, and pertinent laboratory information.
- Assesses patient’s/client’s and families’ knowledge of cardiovascular disease and/or diabetes including individual risk factors.
- Develops a Plan of Care relative to weight, blood glucose, and lipid management based on thorough assessment.
- Determines client outcomes based on interview, laboratory, dietary, and medical history information.
- Develops a plan for individual instruction to patients/families based on nutrition for good health.
- Designs educational programs on nutrition, diet therapy, and weight loss for patients and community groups.
- Implements the Plan of Care for improving patient/client management.
- Communicates the plan of care with patients and/or families, including desired outcomes.
- Demonstrates competent instruction and counseling techniques.
- Works in collaboration with the patient and health professionals toward implementation of the plan of care.
- Provides individual and group educational classes.
- Evaluates the Plan of Care, documenting patient/client response to diet and adherence to protocols.
- Collaborates with interdisciplinary staff in monitoring progress toward nutritional goals.
- Adjusts teaching to accommodate individual needs.
- Documents clinical findings that require medical follow-up.
- Completes patient Progress Letter describing outcomes from participation in programs.
- Demonstrates competence in monitoring a wide range of patients/clients.
- Develops a teaching plan for cardiovascular and/or diabetes education.
- Exhibits a professional demeanor and completes assignments with accountability.
- Functions as a preceptor for new staff.
- Participates in community events and departmental quality management programs.
Education:
- BS Degree from an approved college or university and a coordinated undergraduate program;
Registered Dietitian.
Experience:
- Minimum of two years experience in clinical dietetics.
Licensure/Certification:
- Registered Dietitian. Must maintain registration with the Commission of Dietetic Registration.
- Must be a Licensed Dietitian (LD) in the State of New Hampshire.
