Overview
We are seeking a Community Care Coordinator for our senior services program.
Schedule
:
Full time 40 hours a week Monday-Friday
Compensation
: $62,500 annually
- Assess needs of at‑risk individuals who have spent excessive time in Urgent Care and/or the use of other emergency services, including unmet needs, service gaps and barriers. Serve as a “one stop” resource for coordination of care. Assist in accessing necessary services.
- Work in collaboration with other service providers to maximize use of existing resources.
- Coordinate care delivery mechanisms with client, caregivers, providers and others.
- Complete assessments that may include, but are not limited to, outreach progress notes, intake assessments, case progress notes, care plans, data collection, and provide positive outcome summaries.
- Participate in all relevant meetings, conferences, and sub-committees as assigned.
- Educate service providers and others about specific issues that may be impacting client s needs.
- Establish rapport with other care providers and facilitate meetings as needed to identify and resolve unmet needs, service gaps and barriers. Strategize possible systems change solutions to services.
- Access ancillary service providers and preexisting programs whenever possible; provide services directly when no other resources are available or when direct service from Community Care Coordinator is indicated.
- Maintain a positive working relationship with both inter- and intra-Agency sources.
- Maintain professional ethics and adhere to the NASW code of ethics.
- Retain state certification and/or licensure as applicable and continue to develop professional skills by attending in-services and conferences.
- Master’s degree required, in Social Work or related field
- Five (5) years’ experience providing social work services to the respective community
- Demonstrate a genuine interest and understanding in working with the at-risk populations
- Possess ability to function as an integral member of a multi-disciplinary administrative team
- Valid driver’s license, auto insurance, and reliable transportation
