The VNA of Manchester and Southern New Hampshire provides 24/7 in-home support and services to patients in Manchester and the surrounding communities. We are committed to helping our patients navigate the challenges they face at home while recovering from surgery or coping with life-limiting illnesses or physical disabilities. The VNA care team provides individualized care across the continuum, bringing peace of mind to patients and their loved ones while providing professional healthcare services in home settings.

Demonstrates competence in providing professional speech/language pathology services to clients/patients of all ages whose needs range from basic to complex. Plans and implements age-appropriate, specific treatment programs for individual client/patients with a level of skill in accordance with accepted principles and practice.

• Assess patients and environment appropriately.
• Plan and implement care appropriately.
• Evaluate and update the appropriateness of the care plan and physicians’ orders.
• Communicate appropriate information to patients, peers, managers, physicians, departments, and other agencies involved.
• Integrate problem-solving methodology and quality improvement in resolving problems related to patient care and organizational processes.
• Explain insurance and costs of care appropriately to patients and their significant others.

• Education: Master’s degree or equivalent in the field of speech/language pathology required.
• Experience: Equivalent of one (1) year in a staff speech/language pathologist position in a relevant work setting required.
• Certification/Licensure: Certificate of Clinical Competence from the American Speech-Language-Hearing Association required. Licensed by the Board of Speech-Language Pathology required.
• Other: Valid Driver’s License required to travel throughout southern New Hampshire to provide home health care service and other job-related activities. Proof of automobile insurance required. Motor vehicle background report will be conducted.
