The Greater Manchester Region has approximately 2,500 employees. It includes Manchester Memorial Hospital, a 249-bed community hospital, Rockville General, a campus of Manchester Memorial Hospital, a 102-bed facility, a large multispecialty provider group and visiting nurse services. The Greater Manchester Region serves a region of 300,000 people in 19 towns.

This position reports to the Adult Ambulatory Coordinator and/or to the Director of the Service and the Medical Director. Provides direct and indirect clinical services to adults in need of mental health care and is responsible for treatment of the client and provision of age-appropriate care. Core skills may include crisis assessment, intake assessment, individual therapy, group therapy, family therapy, treatment planning, case management, team/community collaboration, patient/family education and triage/discharge planning.

As skill sets permits, may provide flexible coverage to another service within Behavioral Health on a short-term basis. Creates a caring and compassionate environment through the Core Values of Teamwork, Communication, Respect and Caring.

COMPETENCIES

  • This position requires computer competency with basic knowledge of Microsoft Word and Outlook, strong clinical skills including a demonstrated team orientation, and strong written as well as verbal communication skills using the English language.
  • Must be flexible in approach so that service- specific as well as overall system needs can be met.
  • Must be able to function effectively and, at times, independently in an environment where multi-tasking is necessary.

  • Generates and disseminates accurate clinical case formulation and information and identifies realistic treatment goals, as evidenced by documentation on treatment plans and in clinical team meetings and individual and/or group supervision.
  • Conveys warmth, genuineness and empathy and respects the patient as a whole person with strengths and needs, as evidenced through direct observation and patient feedback.
  • Establishes a shared sense of outcome and processes goals with the patient, as evidenced by patient feedback and direct observation.
  • Consistently obtains required releases of information and networks, communicates and coordinates with community providers—Department of Children and Families (DCF), Adult Probation, etc., during treatment and through discharge, as evidenced by chart audit.
