Job Summary

The MSW Case Manager leads the complex discharge planning process and collaborates with physicians and the healthcare team to assess, plan, implement, and evaluate proactive, complex, and safe discharge plans. The role collaborates with RN Case Managers for guidance on medically complex cases, facilitates coordination of care, and serves as a resource for psychosocially complex cases. The case manager maintains knowledge of community resources and post‑acute care options while aligning with UVA Health policies, mission, vision, and values.

Responsibilities
  • Communicate effectively with patients, guests, and all levels of authority.
  • Provide communication and updates to patients and guests, ensuring their needs are met.
  • Respond quickly and appropriately to irregular and emergency situations.
  • Review processes, identify and recommend improvement opportunities.
  • Monitor quality of core public safety processes.
  • Hold subordinates accountable for customer service standards and initiate remediation when needed.
  • Maintain confidentiality in all matters related to patient care and visitor issues.
  • Actively participate in identifying opportunities to improve patient and guest experience.
  • Provide cultural information to facilitate support for a treatment plan.
  • Ensure that information related to the patient’s outpatient services, hospitalization, advance directives, consents, diagnosis, prognosis, treatment plan, follow‑up appointments, and health care education are accurately communicated.
Qualifications
  • Education:

    Master’s Degree in Social Work from a school accredited by the Council on Social Work Education.
  • Experience:

    2+ years of professional social work experience or healthcare experience; case management experience preferred.
  • Licensure: N/A.
  • Additional Skills/Requirements:
    Weekend and holiday coverage; knowledge/competence of Microsoft Office and keyboarding preferred.
Physical Demands
  • Frequent sitting, standing, walking, bending/stooping, repetitive body movements.
  • Ability to lift/push/pull up to 20 lbs. occasionally.
  • Adequate communicative, auditory, and visual skills; attention to detail; ability to write legibly.
Other
  • May require the use of safety equipment such as a HEPA mask for infection prevention.
  • On‑call responsibilities as directed.
  • Ability to travel between campus buildings, remote facilities, and out of town as needed.
