The role you'll contribute

Promotes the psychosocial health and wellbeing of members enrolled in the care management program, through member assessment and coordination of resources. Provides assessment and consultation for members and facilitates coordination and communication between the member, family, as appropriate, and health care and community resources. Works collaboratively with nurse care managers, physicians, staff and other health care professionals within the care team to provide care coordination across the health care continuum.

Serves as an integral member of the health care team who works to ensure members with complex issues or high utilization, navigate through the health care continuum while improving member/family experience and eliminate care gaps through addressing the social determinants of health. Coordinates a wide range of community-based and healthcare support services for members.

  • Counsels and assists members enrolled in care management with health-related social determinants, financial, emotional and social issues.
  • Provides assessment of members behavioral health, medical and psychosocial needs and develops care plan with goals appropriate to the assessment findings.
  • Provides members and caregivers education on community resources, facilitates referrals as appropriate, and monitors outcomes of referrals.
  • Monitors patient and family/caregiver functional and psychosocial status and advocates for patient needs
  • Completes consultation requests made via referrals from RN Health Advisor, Healthcare Coordinators, pharmacists or other eligible sources in a timely manner
  • Actively participates as a member of the collaborative care team
  • Demonstrates commitment to organizational and team success by maintaining attendance and productivity standards
  • Provides intervention and/or community linkage for patients and families who are experiencing significant emotional, social, environmental, or financial stress due to hospitalization, acute or chronic illness and/or who need help in meeting their continuing care needs.
  • Provides thorough, timely, and appropriate documentation, in accordance with professional standards, in care management system.
The expertise and experience you'll need to succeed

Minimum qualifications:

  • Master'sDegree
  • 3 years work experience
  • Clinical Social Worker License (LCSW)
  • Licn Mental Health Counselor (LMHC)
