Position Overview
- Role:
Nurse Practitioner (NP) or Physician Assistant (PA) - Location:
Maine - Schedule:
Full-time (4 or 5 days per week) with no weekends, holidays, or overnight shifts - No weekends, holidays, or overnight call
- Compensation: $90/hour All travel and housing covered
- Care Model:
Primarily onsite with some telemedicine coverage
What You’ll Do
- Provide comprehensive geriatric medical care in assisted living, memory care, and long-term care facilities
- Manage chronic conditions, acute issues, and regulatory visits with a patient-centered approach
- Develop and oversee individualized care plans focused on function, safety, and quality of life
- Collaborate closely with nursing staff,facility leadership, and interdisciplinary care teams
- Participate in telemedicine visits to enhance continuity and access to care
- Use remote patient monitoring tools to support proactive and preventive care
- Complete timely, high-quality documentation in the EHR
What You Bring
• Active NP or PA license in the state of Maine
• Experience in geriatrics, long-term care, or post-acute care—or a strong interest in this specialty
• Comfort working independently while collaborating within a team-based model
• Strong clinical assessment, diagnostic, and decision-making skills
• Confidence using technology, EHRs, and telehealth tools
• Clear, compassionate communication with patients, families, and care teams
