Case Manager (862) – Lifeworks

Location:

West Roxbury, MA

Shift: Monday – Friday, 8:30 am – 4:30 pm
Salary: $50,473 per year

Essential Duties
  • Promote optimal health through routine, preventive, and emergency health care; ensure safe environments and life enhancement activities.
  • Provide supervision, training, and coaching across all settings to increase self‑sufficiency and independence.
  • Offer education, training, counseling, guidance, and physical assistance to help individuals achieve goals in employment, education, social growth, health and wellness, communication skills, community mobility, and volunteerism.
  • Develop, write, and present annual Individual Support Plans (ISPs).
  • Perform case‑management duties: document training and activities, maintain confidential files on assigned caseload, submit required paperwork to supervisor, and attend regular meetings.
  • Maintain regular communication with individuals and all ISP team members to facilitate consistency of programming and support.
  • Provide transportation to and from community sites using Lifeworks or personal vehicles; reimbursed at the agency mileage rate.
  • Demonstrate good computer skills and ability to learn agency computer programs for documentation.
  • Complete other duties or assignments as needed.
Requirements
  • Experience with individuals with /DD.
  • Bachelor’s or associate degree in a related field with two years of experience, or high‑school diploma and five years of related human services experience.
  • Pass fingerprint, CORI, DPPC, and driving background checks.
  • Valid driver’s license.
  • Valid MAP certification preferred.
  • Excellent verbal and written communication skills.
  • Basic computer literacy and willingness to learn new systems.
