Community Nursing Teams provide interventions for housebound patients aged 18 and over, delivering personalised nursing care to those with complex and long‑term conditions. The aim of our service is to enable patients to stay safe and well in their chosen environment while we work to prevent avoidable admissions through tailored care that supports their long‑term health goals and self‑care wherever possible.
The service operates in teams aligned to Primary Care Networks and provides integrated care founded on the framework of the Multidisciplinary Team.
- Palliative and end of life care provision including symptom control, advance care planning and verification of death
- Administration of medication including IV therapy
- Wound management including leg ulcers, post‑operative surgical wounds, drains
- Bladder and bowel care
- Health promotion, education and support to manage long‑term conditions

Driving requirementMany of our roles require the ability to drive. PersonSpecification Qualifications, training and experience
- You will have a Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing.
- You will be a registered health care professional with the Nursing and Midwifery Council.
- You will have recent experience of healthcare gained through education, work experience or previous employment. This role is ideal for a newly qualified graduate nurse.
- Full UK Driving Licence with a car.
- You will have sound communication skills in order to understand and communicate both verbally and in writing.
- You will be able to use IT systems and databases.
- You will fully understand the role and remit of a health care worker.
- You will have knowledge of up‑to‑date clinical practice using evidence‑based procedures and clinical effectiveness frameworks.
- Having an awareness of developments within the NHS would be an advantage.
- A full, valid UK driving licence and the ability to travel independently are essential for this role. Should you require workplace adjustments under the Equality Act 2010, we are committed to supporting you to meet the mobility requirements of the role wherever possible.
