We're looking for an experienced CPN ready to hit the ground running and manage the complexities of a shared community caseload in Maidstone's Mental Health Together PLUS Team to cover maternity leave.
You will be required to work autonomously, with the support of peers and Leads, to ensure the safe and effective care of clients sat within shared caseloads. You will be required to cover duty at times.
The environment is fast-paced and highly unpredictable; therefore a level of resilience and organisation is required.
The skills gained from working within a community team are vast, and this is an ideal opportunity for an experienced nurse looking to try something different.
The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
Main duties of the job- You will carry out and coordinate comprehensive, systematic nursing assessments and reviews which take account of relevantmental, physical, social, cultural, psychological, spiritual, and environmental factors, in partnership with the service user and their network, to agree a holistic person centred care plan that addresses the individual's needs.
- The post holder will need to undertake crucial screening and assessments, facilitate appropriate referrals to mainstream primary and secondary mental health services, and act as an advocate for clients in the referral process andoffer continued support and brief interventions with a view to maintaining/enhancing engagement.
- To act as a leader, supporting best practice in line with NICE guidance.

Person Specification Training, Qualifications and Registration- Registered Mental Health Nurse
- Evidence of additional training in therapeutic interventions
- Previous experience of recovery focused service provision with individuals who have complex mental health needs.
- Additional / advanced physical health training
- Excellent written communication skills to enable completion of records etc.
- Comprehensive understanding of, and ability to apply, relative frameworks
- Ability to work without direct supervision and prioritise workload effectively.
