Position: RN Resource PM Shift (Verona)

RN Resource PM Shift (Verona)

Location:

Verona, WI.

Drive up to 50 miles on average from your home.

Part‑time 0.6 FTE (24 hours per week). PM Shift 1600‑0030.
Includes on‑call rotation (1 NOC shift/month), weekends (every other weekend), and holiday rotations (3 per year).

Sign on bonus: $10,000.

Key Responsibilities
  • Patient and Family Assessments
    • Assess patients and families' physical, pain, psychosocial, emotional, spiritual, environmental, and educational needs per the plan of care.
    • Collect data from patients, families, significant others, and interdisciplinary team members.
    • Identify, interpret, and document assessment data captured in the nursing interview, physical assessment, and other health data.
    • Document and report significant changes in condition or unusual incidents to appropriate IDT member (e.g., abuse, suicidal intent, violence).
    • Collaborate with IDT and primary care provider to create HES for hospice eligibility determination.
  • Patient Visits
    • Respond to patients and families' needs or changes in condition requiring acute intervention.
    • Provide interventions, treatments, and pain management to meet needs.
    • Evaluate effectiveness of pain management and adjust accordingly.
    • Provide education on pain, symptom management, end‑of‑life, and evaluate effectiveness of education.
  • Plan of Care
    • Formulate, implement, and revise individualized POC with IDT, patient, and family.
    • Develop reasonable outcome measures and set priorities in care plan.
    • Follow and document interventions, outcomes, and changes in patient condition in the electronic health record (EHR).
    • Delegate visits to appropriate disciplines and manage visit sets.
    • Oversee work of Certified Nursing Assistants and Licensed Practical Nurses.
  • Miscellaneous
    • Participate in meetings to plan and evaluate patient/family services.
    • Precept new staff and students.
Qualifications
  • Current, unrestricted RN license in state of employment.
  • Preferably one year of nursing experience.
  • Valid driver’s license, auto insurance, and reliable vehicle.
  • At least 18 years of age.
  • Basic Microsoft Office skills.
  • Intermediate electronic health record skills;
    Epic preferred.
  • Ability to use proper body mechanics, safe patient handling, and standard precautions.
