Position details: Full Time 0.9 FTE (36 hours/week) | NOC Shift | Weekdays | 8:00pm-8:30am
Includes on-call rotation (1 NOC Shift per month), weekend rotations (every 3rd weekend), and holiday rotations (3 per year).

How You’ll Make a Difference (Essential Functions)
The Registered Nurse Alt Shift (RN Alt Shift) is responsible for patient and family satisfaction through nursing care and case management, including assessments, triaging, planning, interventions, communication, and documentation. The RN Alt Shift provides coverage for care groups across homecare teams as needed (vacancies, FMLA, high census). Care is delivered in alignment with Agrace philosophy, goals, standards, regulations, and policies. Percentages of time for each function may vary based on needs.
Key Responsibilities
- Patient and Family Assessments
- Assess patients and family needs (physical, pain, psychosocial, emotional, spiritual, environmental, educational) per plan of care.
- Collect data from patient, family, and interdisciplinary team members.
- Document and report significant changes or incidents to the appropriate IDT member.
- Collaborate with IDT and primary care provider to determine hospice eligibility.
- Patient Visits
- Respond to patient/family needs or changes in condition; provide interventions and pain management.
- Evaluate effectiveness of interventions and adjust as needed.
- Educate patients and families on pain management, symptoms, and end-of-life care; assess effectiveness of education and adjust.
- Plan of Care
- Develop, implement, and revise individualized POC in collaboration with IDT, patient, and family.
- Set achievable outcome measures and priorities; follow the plan of care and document in the electronic health record (EHR).
- Delegate visits and manage visit sets; oversee CNA and LPN work.
- Miscellaneous
- Participate in meetings to plan and evaluate patient/family services; precept new staff and students.
You Bring (Qualifications)
- Current, unrestricted Registered Nursing License in the state of employment
- One-year nursing experience, preferred
- Valid driver’s license, auto insurance, and access to a reliable vehicle
- Must be at least 18 years of age
- Basic Microsoft Office skills
- Intermediate electronic health records skills;
Epic preferred - Ability to use proper body mechanics, safe patient handling, and standard precautions
