• Provide thoughtful care to residents based on personalized Plane of Care.
• Monitor and track changes in residents' conditions and needs.
• Proactively update the appropriate contacts regarding residents’ plane of care, health changes, and concerns.

• Previous experience in nursing or other medical fields.
• Familiarity with medical software and equipment.
• Ability to build rapport with patients.
• Strong problem solving and critical thinking skills.
• Ability to thrive in a fast‑paced environment.
• Current RN license.
