Shift Schedule- 3:00 PM - 11:00 PM
- 11:00 PM - 7:00 AM
Walk-In Interviews:
Monday – Friday, 9:30 AM – 2:30 PM
Madison Health and Rehabilitation Center is a 60-bed Skilled Nursing Facility offering both private and semi-private rooms. We specialize in short-stay rehabilitation and long-term care, and we are proud to be a deficiency-free facility

- Care for multiple residents in the facility. Observe Resident Rights.
- Provide personal care and assistance with activities of daily living.
- Assist nurse with admitting, transferring, and discharging residents.
- Assist nurse in giving treatments and examining residents.
- Assist with restorative and rehab care with proper instruction and supervision.
- Come to work as scheduled and consistently demonstrate dependability and punctuality.
- Certified Nursing Assistant (CNA) Required
Education and Experience: - Current and valid Certified Nursing Assistant (CNA) license in the state of practice and employment.
- Ability to read, write, speak, and understand the English language.
- Ability to make independent decisions when circumstances warrant such action.
- Ability to work harmoniously with other personnel.
