This is a full-time hybrid role (40 hours per week). Candidates must be residents of Wisconsin.
Under the direction, guidance and general supervision of the Population Health Manager, the Population Health Specialist plays a crucial role in coordinating and supporting GHC-SCW’s population initiatives, community partnerships, community health programs, and social work services. They actively contribute to various community health programs, including the Bridges to Access Program, Stay Healthy Visit Program, Refugee Assistance Program, Primary Access for Kids Program, and Financial Assistance Program.
In addition to program involvement, the Population Health Specialist aids in the development, monitoring and sustainment of partnerships and charitable contributions. The position actively supports the organization’s health equity initiatives for both members and patients. This position is also a key contact for GHC-SCW clinic staff that assist in identifying and directing patients to other community services and resources that may be available.

Job Specific Minimum Qualifications
- Bachelor’s degree from an accredited college or university in a relevant major, or equivalent combination of relevant education and experience is required.
- A minimum of two (2) to three (3) years of experience in an office or healthcare environment including customer service is required. Direct experience with community services and/or Medicaid or Badger Care population is preferred. Previous experience using the telephone in a service capacity is preferred. Experience with medical terminology is preferred.
- Knowledge of and proven proficiency in the use of Microsoft Office applications including Word, Outlook, Excel, PowerPoint, Visio is required. Proficiency in SharePoint is preferred.
- Possession of current State of Wisconsin Driver license with a good driving record, access to a vehicle, and the ability to travel is required.
- Ability to work with a diverse patient population is required.
- Ability to occasionally work evenings, weekends, and off hours, as required.
- Excellent customer service skills are required. Ability to be culturally sensitive and approach members and others in a non‑intimidating and supportive manner, and the ability to represent GHC‑SCW in a professional and compassionate manner is required.
- Knowledge of or ability to learn all community resources, agencies and referral sources which are appropriate for member population groups required.
- Knowledge of and/or ability to learn aspects of the Medical Assistance and Badger Care programs and the ability to learn GHC‑SCW’s services and benefits are required.
- Ability to learn and effectively use multiple computer systems including electronic medical record and scheduling systems (e.g., Epic Systems) is required. Epic experience preferred.
- Excellent verbal and written communication skills and the ability to document, design, and create meeting materials, general correspondence, memos, charts, tables, graphs, web page content, reports, etc., required.
- Outstanding customer service skills are required.
- Knowledge of or ability to learn about healthcare delivery systems including insurance and managed care. Ability to keep abreast of population health trends and tactics.
