Position: Home Coordinator Caregiver

Under the supervision of the Residential Case Manager (RCM) and the Director of Operations (DOO), the Home Coordinator is responsible for providing direct support to consumers in their homes and community, and ensuring the smooth operation of the home and team. The HC helps RISE UP fulfill its mission to enable consumers to live independently in the community, achieve full membership, and receive support throughout their lives in the community setting.

JOB DUTIES AND RESPONSIBILITIES:

75% Direct Consumer Support:
  • Plan, support, and accompany consumers in activities such as home upkeep, grocery shopping, banking, cooking, attending church, coffee shops, health clubs, volunteering, employment, social outings, etc.
  • Provide personal care, including assistance with clothing, dressing, bathing, and grooming.
  • Assist with meal planning, shopping, and food preparation.
  • Support household chores like laundry, cleaning, and errands.
  • Help consumers develop and maintain relationships with friends and family, tailored to individual needs.
  • Assist with hospitality in homes and neighborhoods, including hosting.
  • Monitor health and medication needs.
  • Support effective communication of needs and wishes.
  • Implement plans developed collaboratively with the team, medical professionals, and guardians.
  • Promote consumer choice, independence, and inclusion, while fostering positive perceptions and focusing on abilities.
15% Home Management:
  • Arrange transportation, including scheduling rides and cancellations.
  • Manage grocery shopping, ensuring a supply of fresh food and beverages, with at least two weeks of supplies maintained.
  • Monitor and maintain home paperwork, including timesheets, medication sheets, logs, consumer information, and personal funds, keeping at least two weeks of documents in stock.
  • Oversee medical supplies and equipment, alerting the office before supplies run low.
  • Review medication administration records for accuracy and report errors.
  • Serve as liaison between the office and house schedules.
  • Assist with planning appointments, birthdays, and social events.
  • Support participants in hosting parties or communicating with family and friends.
  • Attend medical appointments with the RCM and transport participants to non-critical appointments once they are approved drivers.
  • Ensure the home functions smoothly at all times, including planning for absences or time off.
