• Record information concerning inquiries, admissions, and discharges and assist with daily census reports.
• Coordinate the inquiry process and facilitate admissions, including completing required paperwork.
• Notify facility representatives of admission updates.
• Maintain monthly census records, matching numbers with Business Office for accuracy at month end.
• Provide information on referral leads, admissions, and discharges on a weekly basis or as requested.
• Communicate effectively with Social Security Department, Nursing and Business Office regarding bed availability.
• Evaluate referrals to determine admission candidacy, including home visits or community agency involvement.
• Maintain close contact with all referral sources.

• High School Diploma or equivalent.
• Admissions experience in LTC/SNF preferred.
• Positive, energetic individual with creativity.
• Ability to work independently or as part of a group.
• Computer knowledge.
