The Role (Key Responsibilities)
As our Registered Nurse (RN) Case Manager, you will be responsible for making a difference and driving impact. Your daily impact will include:

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit and regularly reevaluates the patient's needs.•
• Initiates the plan of care and necessary revisions.
• Prepares clinical and progress notes for each patient visit in a timely manner as per agency policy.

What You Bring (Qualifications)

• Experience: 2 years of experience in [Field/Industry].
• Education: RN License, BSN preferred.
• Skills:
  • Ability to exercise initiative and independent judgment.
• Soft Skills: Strong communication, problem-solving, and train other associates
• Certifications: BLS
