LPN/RN will work closely with Adult Day Team. This job requires excellent communication and organizational skills, must have an active GA LPN/RN license. Must enjoy working with the elderly and disabled. Must have good documentation skills. Must be proficient in Microsoft Office. Medication administration and assessments required.
Job Duties- Collaborate with healthcare providers, case management agencies and Area on Aging
- Daily assessments of clients. Monitor clients for signs and symptoms.
- Document daily census and nursing assessments.
- Communicate with clients, families and staff.
- Intial assessments of new ADH clients.
- Monthly assessments
- Facilitate the exchange of medical information between different healthcare providers as needed
- This is not a hybrid position. Position is at the Adult Day Center.
