Responsibilities

The Registered Nurse (RN) in Home Health will perform patient visits, coordinate care, and serve as the primary contact for all disciplines involved in patient care. Responsibilities include consulting with physicians, performing accurate OASIS data collection, maintaining current medication profiles, reporting lab values timely, and participating in weekly case conferences and monthly case manager meetings.

Qualifications

Registered Nurses must meet the following requirements:

• Current RN license in the state of employment
• At least one year of clinical experience preferred
• Knowledge and skills in current nursing practices
• Valid driver’s license and automobile liability insurance
