Clinical Coordinator
We are seeking a Clinical Coordinator to assist with all clinical needs for our Macon patients. The Clinical Coordinator plays a crucial role in ensuring the seamless coordination of patient care, including chart preparation, communication, scheduling of patient visits, and logging of chronic care management activities.
This position will be working in the Macon office daily. It is a full‑time, salary‑based 8‑hour position (8:00 AM–5:00 PM, Monday‑Friday).

- Review and organize patient medical records in preparation for visits.
- Ensure all necessary documentation, test results, and relevant information are accessible for the provider. Utilize Navina to manage these entries.
- Maintain accurate, up‑to‑date patient records, including demographic, contact, insurance, and appointment history.
- Ensure confidentiality and adhere to privacy regulations. Verify active consent forms and alignment paperwork are completed and in the patient’s chart.
- Reconcile medications.
- Collaborate with the care team to schedule and coordinate patient appointments with the company’s provider(s) and other team members.
- Communicate with patients via email, patient portal, telephone, text, and in‑person to gather information, schedule and confirm appointments, and provide pre‑appointment instructions.
- Utilize communication methods such as phone, email, Athena Text, Microsoft Teams, etc.
- Appropriately and accurately document and log care management activities.
- Management may assign new duties, reassign existing duties, and/or eliminate functions as necessary.
- Must be a Certified Medical Assistant (CMA) or licensed nurse (LPN or RN) and licensed in the state of employment. Certification/license must be in good standing.
- A minimum of one (1) year of experience in a healthcare setting is preferred.
- Must hold and maintain a current CPR certificate.
- High school diploma or equivalent required.
- Strong interpersonal and communication skills under all conditions.
- Ability to foster a cooperative work environment.
- Team player with the capacity to manage multiple responsibilities and demonstrate sound judgment.
- Excellent written and verbal communication skills.
- Basic computer knowledge.
- Must be able to work flexible hours and travel between offices, facilities, etc.
- Must be a licensed driver with an insured automobile in good working condition.
