The Ambulatory Care Clinical Pharmacist Specialist will provide comprehensive medication management that optimizes patient outcomes. This includes pharmacist-managed chronic disease state management, transitions of care communication, and other direct patient care activities by designing, recommending, monitoring and evaluating patient specific pharmacotherapy. This individual will collaboratively practice with Primary Care physicians and other clinicians in the development of appropriate patient care plans, patient education and required patient monitoring and follow-up as necessary

Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Evaluate, monitor, recommend and design patient specific pharmacotherapeutic regimens, in collaboration with primary care physicians, including but not limited to patients with diabetes, hypertension, congestive heart failure, COPD and asthma.
  • Provide drug-therapy related education and training to team members
  • Demonstrate knowledge and communication skills to effectively counsel patients and respond to patient questions.
  • Coordinate and evaluate appropriate laboratory tests and utilize physical assessment skills necessary to monitor and support the patient's drug therapy plan.
  • Appropriately documents directly in the patient's chart any medication-related assessment and plan of care that follow acceptable standards for documentation and billing if applicable.
  • Identify drug therapy strategies that improve patient outcomes and reduce costs
  • Precept pharmacy students as needed.
  • Participate in committees and meetings to facilitate improvement of patient care and development of goals, programs and objectives for specialized patient populations
  • Ability to work collaboratively as a member of an interdisciplinary team, establish and maintain effective working relationships with other healthcare professionals, practitioners, patients and families.

EDUCATION AND EXPERIENCE

Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited Doctor of Pharmacy degree program OR possess a Bachelor or Master of pharmacy degree with 3 years of pharmacy experience.

ASHP-accredited Pharmacy Practice Residency preferred (or equivalent one year pharmacy experience working in the ambulatory setting if residency has not been completed)

CERTIFICATION AND LICENSURE

Licensed (or eligible for licensure) in the State of Illinois and/or Wisconsin based on practice setting

Approved CPR or BLS certification required within 90 days

Board of Pharmaceutical Specialties certification or Certified Diabetes Educator (CDE) certification preferred as deemed appropriate and relevant to the practice.
