Overview
Intensive Case Coordinator position requires organizing, mediating and documenting Child and Family Team meetings, assisting families with the CFT process, and providing services in the community/in-home settings as designated by program participants.
Responsibilities- Organize, mediate and document Child and Family Team meetings; assist families with the CFT process as needed; mediate discussions regarding flex fund requests with youth and family; track progress on action steps throughout the treatment process.
- Work as part of a multidisciplinary team including family members, natural supports, DMH, DCFS, Department of Probation, psychiatrists, and other staff to provide consistent services.
- Complete and submit SCHARP check request forms for processing Flex Funds for supervisor review and approval.
- Develop client Safety and Crisis plans in collaboration with the Child and Family Team; coordinate and monitor treatment progress and Safety Plans to align with the child’s and family’s needs, desires, goals and objectives.
- Maintain contact with family members and significant others; involve them as appropriate or at the youth’s request.
- Maintain a UOS log daily for all services rendered; ensure service codes and time billed are consistent; complete documentation and submit within 48 hours of direct service contact; ensure progress notes are completed before billing.
- Involve child/youth and families as partners, providing choices and opportunities for empowerment where possible.
- Assist in establishing goals in living arrangements, social relationships, leisure, productive activity, and health care for the child/youth and family.
- Attend and participate in relevant SCHARP, DCFS, DMH and/or Department of Probation meetings for each client as necessary.
- Be available to respond by phone and in person to crisis calls on an on-call rotational schedule.
- Perform other programmatic and administrative duties as assigned.
- Education:
- High School Diploma or equivalent
- Bachelor’s Degree in Social Work, Psychology, or other human services field
- Experience:
- At least 2 years of experience (3 years without a Bachelor’s degree) working with high risk children/youth in child welfare, probation, or mental health systems
- Bilingual English/Spanish preferred
- Special Skills and Knowledge:
- Ability to respond quickly, effectively and with equanimity to client crises and life situations
- Flexible schedule; ability to provide services in the community/in-home as designated
- Strong verbal communication and ability to implement core practice treatment model
- Excellent written and verbal skills
- Basic computer skills to use the agency electronic health record; ability to log on, type, use a mouse, and navigate screens
- Knowledge of mentally ill child and youth populations, targeted case management and rehabilitation
- Must have valid California driver’s license and access to a car with adequate insurance
