Job Details
- Must have ACLS certification (American Heart Association or Red Cross)
- BLS certification required (American Heart Association or Red Cross)
- License/Certificate Verification with Primary Source Verification of professional license and/or certification required
- Must pass Prime Basic Rhythm Interpretation Screening with a score of 80% or higher
- Must pass Prime Core Competencies Exam with a score of 80% or higher
- Must pass Prime Specialty Exam with a score of 80% or higher
- Prime Skills Checklist completion required
- COVID-19 vaccine (2 doses and booster) or declination required
- Proof of MMR immunity (2 vaccines or positive titer) required
- Annual Fit Test required
- Hepatitis B vaccination (3 doses) or titers required
- Influenza vaccine or declination required (October 1 - May 31)
- TB Test required
- Varicella vaccination or titer showing immunity required
- Tdap vaccination required (declinations not accepted in some locations)
- Physical/Health Statement annually required, must state clear to work
- National Criminal Background Check within one year of start date required
- Prime Form I-9 (Employment eligibility verification) required
- DCI/BVP certification required

Schedule Information
- Prime CA Sexual Harassment Prevention Training valid for 2 years
- Prime Orientation Packet to be completed on first scheduled shift
- Driver's License, State , or Passport required
- Prime OIG and Prime SAM/EPLS checks required
- Prime Resume and two professional references required
- Name Verification Affidavit required if name discrepancies exist
- Positive PPD with Negative Chest X-Ray optional if applicable
