Mental Health Rehab Specialist – St. Francis Medical Center, Los Angeles, CA

Responsibilities- Assess clients, caregivers, and families to identify needs for Case Management and Mental Health Individual Rehabilitation Skills.
- Provide case management linkage services and follow‑up documentation in the department’s electronic health record system.
- Assist in establishing treatment goals and work with clients to improve, maintain, or restore life functioning.
- Deliver Mental Health Individual Rehabilitation Services in the office and, when needed, in the field (home or community locations) to reduce barriers to access.
- Bachelor’s or Master’s degree in Social Work, Psychology, or a related field.
- Excellent verbal and written communication skills with clients, families, physicians, and community professionals.
- Experience with best‑practice case management and mental health rehabilitation skills.
- Experience working with children and families of diverse cultural backgrounds.
- Bi‑lateral proficiency in English and Spanish.
- Computer literacy and valid California State Driver’s License.
- Completion of Behavioral Violence Prevention training within 6 months of hire and maintenance annually (preferred).
Competitive compensation range: $21.00 to $25.71 per hour. Benefits include paid time off, 401(k) retirement plan, medical, dental, and vision coverage, tuition reimbursement, and additional voluntary options.
