The Physical Therapist–PACE position provides services to help restore function, improve mobility, relieve pain, and prevent or limit disabilities in patients with injuries or disease. Responsibilities include evaluating individuals with mobility issues and creating personalized care plans to promote functional independence.

This is a specialized Physical Therapist (PT) position supporting the Rehabilitation Manager across various domains: staff coverage, training, administration, project coordination, and regulatory compliance. The PT may work remotely or in-person at Alta Med PACE Centers as needed, supporting assessments, care planning, and team meetings. The role includes training, data collection and analysis, report building, presentations, and compliance recordkeeping. The PT will assist in program planning, CQI initiatives, and serve on interdisciplinary committees to improve outcomes and service delivery.

Collaboration with managers, QI team, and other leaders is essential to align with organizational goals and ensure excellent patient care.

• California Physical Therapy license with at least 1 year of experience working with the elderly and disabled populations (preferred).
• Current BLS certification.
• Bilingual in English/Spanish/Cantonese/Mandarin is preferred.

$51.26 - $64.08 hourly
